How to build a VA medical timeline

A medical timeline turns scattered records into a clear story. It helps you spot gaps, support a nexus argument, and prepare for a C&P exam or appeal.

Gather your records

Collect service treatment records (STRs), VA medical records, private treatment notes, C&P exams, DBQs, imaging reports, and prescription histories. The more complete your source material, the more accurate your timeline.

Extract key events

Look for dates of diagnosis, treatment, surgeries, medication changes, and symptom reports. Also note when you first reported the condition in service or during a VA exam.

Build a chronology

Arrange events from earliest to most recent. Group by condition or body system. A clear chronology helps you and the VA see the progression of a disability over time.

Example timeline entry

Below is a simplified example for a tinnitus claim. Your actual timeline may include many more events across multiple conditions.

Mar 2010

Reported hearing loss and tinnitus during deployment

Jun 2012

Separation physical noted hearing threshold shift

Jan 2018

VA audiology exam diagnosed bilateral tinnitus

Sep 2022

C&P exam confirmed current ringing and sleep disturbance

Why a timeline matters

  • Shows continuity of symptoms from service to the present.
  • Highlights missing records or exams you may still need to obtain.
  • Makes it easier for a VSO, attorney, or medical expert to review your claim.

How StratForge helps

StratForge Veterans AI extracts dates, diagnoses, medications, and events from your uploaded records and assembles an interactive timeline. You can search across it, ask questions in plain English, and export it for your claim package.
